Retatrutide (LY3437943) is a triple hormone receptor agonist developed by Eli Lilly that simultaneously acts on GLP-1, GIP, and glucagon receptors. In the Phase 3 TRIUMPH-1 clinical trial (May 2026), Retatrutide delivered the strongest published weight-reduction data for any medication to date.

Mechanism of action

Through the simultaneous activation of three metabolic signaling pathways, Retatrutide exerts a synergistic effect on energy balance, insulin sensitivity, and fat loss. In the phase 3 TRIUMPH-1 trial (NEJM 2026), participants receiving the 12 mg dose lost an average of 25.0% of their body weight after 80 weeks. With 9 mg, the loss was 23.7%, with 4 mg 17.6%, and with placebo 3.9%.

Reconstitution: Example calculation

To achieve a working concentration of 10 mg/ml, reconstitute the 20 mg vial with 2 ml of sterile bacteriostatic water (BAC water). Slowly inject the water along the inside of the vial and gently swirl it instead of shaking it to avoid damaging the peptide structure.
